Pre-Admission Surgical Clinic

The Pre-Admission Surgical Clinic is a hospital service that helps to prepare you for your operation. Located at Bluewater Health in Sarnia, an appointment needs to be scheduled up to three weeks prior to your surgery date. If the surgeon’s office has not given you an appointment, please call the clinic at 519-464-4400, Ext. 4419 to book one.

Please bring the following items to your appointment:

All prescription and non-prescription medications (in their original containers with the name and dosage if possible) including eye drops and inhalers;
COMPLETED forms from the surgeon’s office, including a pre-operative history/physical form;
Ontario health card;
Room coverage/healthcare insurance information for Patient Registration clerk;
A support person that may be able to help you with answering questions and receiving information about your surgery if you are unable to do so.

Patients who are unable to sign their own consent form in the doctor's office must be accompanied by their next-of-kin or legal guardian, so they can sign an Informed Consent Form. Upon arrival at the hospital for your appointment, please register with Patient Registration. You should eat, drink and take your medications as usual the day of your appointment unless advised by your physician.

At the clinic, you will have an assessment done by a healthcare provider. You will be advised how to prepare for your operation, what to expect when you come in for your surgery, how to plan for your discharge from hospital and for your postoperative recovery. In addition, you may have blood work, urine testing, an X-ray or EKG, according to your surgeon’s orders. You may meet with a pharmacy technician to review your medications.

Your doctor can also make a referral to the Erie St. Clair Home and Community Care Case Manager here at the hospital. Plans can then be made for your post-discharge needs before you actually leave the hospital. This may include dressing changes or suture removal.
