The Canadian Partnership for Stroke Recovery and staff from the Calgary Early Supported Discharge Program developed educational videos and easy to use tools to help manage some impairments that may arise as a result of stroke. 


This checklist tool was developed by Heart & Stroke to provide tips for patients, families and caregivers who are transitioning home after a hospital stay.  The tips and information align with the Canadian Stroke Best Practice Recommendations.  It offers guidance and how you can be involved in your care and recovery.
