Assertive Community Treatment (ACT) provides treatment, rehabilitation and support services for individuals with severe and persistent mental illness in Sarnia-Lambton who require intensive service to live independently in the community. Individualized services that address mental and physical health, addiction, education, vocation, social development/functioning, activities of daily living and family, and other relationships, are provided to the client in their homes and the community.
Using an integrated approach by a multidisciplinary team of mental health professionals, ACT works with clients to reach their individual goals and lead healthy lives.
